Free Discovery Call
All patients start with a free discovery call virtual visit* to answer questions and make sure this is a good fit. The next step is to set up an in person new patient consultation. Thereafter, together we will decide upon the frequency of follow-up visits and length of treatment. We see patients across the state of Connecticut, ages 14 and up. The rates listed are market rates. I offer a reduced rate for those in need.

New Patient Consultation
In this comprehensive initial session you will have ample time with your lifestyle medicine provider to go into your medical history, engage in a thorough assessment of your health patterns and behaviors, and discuss your health concerns and goals. Together we will come up with a personalized treatment plan to begin your journey towards improved health. In person only.

Follow Up Visit
We revisit your health goals and assess the progress of your plan, reviewing your successes and any barriers. Success is celebrated and challenges are viewed as growth opportunities. We may tweak your plan depending on your needs. In person or virtual.

Group Visit - Coming Soon
A group visit is a wonderful way to experience the transformative power of lifestyle medicine by combining the expertise of the provider with peer support and learning from others with similar health challenges. Group visits foster trust in a non judgemental and supportive space. Topics may include nutrition and meal planning, exercise, restorative sleep, cooking class, stress management and smoking cessation.

Home Visit
A home visit provides an in depth view of your health habits in the intimacy of your own space. This visit is for established patients who want more support with lifestyle changes tailored to your needs, or if you have a medical condition that prevents you from being able to have an in person or online visit. Examples include, kitchen assessment for healthy eating and cooking, teaching kitchen on healthy food preparation, how to make your home a stress free sanctuary, optimal sleep set up.

Grocery Store Visit
Learn how to navigate the grocery store and select healthy foods with ease and confidence! Topics covered include planning ahead, shopping on a budget, label reading and more.

Pricing
At Lifestyle Medicine New Haven we believe that there is enough for everyone everywhere to have a healthy and productive life. We envision a new health currency that is based on abundance versus scarcity, and that views health, not just through the lens of the individual, but connected to our families, communities and our planet.
Our goal is to offer these services to people, regardless of income. We provide the opportunity for those who have more access to wealth to give more to help support others who have less access to financial resources. We also provide a reduced rate for those in need to make the services more affordable.
In the spirit of reciprocity - when we give, we also receive; when we receive, we are inspired to give. We provide services to lower income patients at a significantly reduced cost. Please consider making a contribution to help us make healthcare equitable!
Payment
Payment for all services may be made by cash, credit card, or check. Full payment is due before or at the time of your visit. Payment plans are offered for the 3 month lifestyle package.
No insurance is accepted at this time, however, we hope to in the future. You will be provided with a super bill for services that you submit to your insurance company, flexible spending account (FSA) or health savings account (HSA) for reimbursement. Lifestyle Medicine New Haven does not submit payment to your insurance company or guarantee reimbursement. This allows us to spend more time focused on your care.
